Information for Patients Attending Physiotherapy
Physiotherapy naturally involves close interaction between the physiotherapist and the patient. During your consultation, you will be asked detailed questions about your condition, including its location, severity, nature, how it began, possible causes, other symptoms and any relevant past medical history.

Physical Examination and Assessment
A physical examination will follow, which may involve undressing the affected area. Please wear suitable undergarments and a vest style top for upper body problems, or shorts for lower body and spinal problems. Towels are available to maintain your modesty upon request. You are welcome to have a partner or friend present during the examination and treatment if you wish.
The purpose of the examination is to rule out unrelated conditions and clarify your diagnosis to determine appropriate treatment options. As your condition may evolve, assessments may be repeated during subsequent appointments. Treatment options will always be discussed with you, and your consent and cooperation are essential at every stage.

Your Comfort and Concerns
While physical manipulations and assessments are routine for physiotherapists, we understand they may cause concern. Please feel free to ask for clarification or request to stop at any time if you feel uncomfortable.

Treatment and Potential Risks
Treatment may involve manual therapy and manipulation techniques, which carry minimal risks. Your safety and well-being are our top priorities, and any concerns can be discussed with your physiotherapist. Please advise your physiotherapist if you have a pacemaker.

Appointment Cancellations
A discretionary cancellation fee may apply if appointments are cancelled with less than 24 hours' notice or if you arrive more than 15 minutes late.

Payment
Payment for treatment may be made by cash, contactless card/phone, bank transfer or via online payment link. If you are attending through private health insurance, patients are kindly requested to check the conditions of their health insurance policy, as the physiotherapist can not be held responsible for any non-payment of claims.

Record-Keeping and Data Protection
We are legally and professionally required to maintain accurate records of your physiotherapy care. This includes demographic details, notes on your symptoms, medical history, and any medications you use. These records help your physiotherapist make informed clinical decisions.

Please be aware that clinical audio of your session may be recorded using secure note-taking software to assist with accurate documentation. There is no way to hear or reproduce the actual voice recording, and only clinical data in the form of text will be exported by the software. You may choose to decline the use of this software at any time, and your care will not be affected.

Your records will be securely stored in line with laws on data protection and confidentiality, either electronically or as hard copies. These are held for eight years after your treatment concludes, after which they will be securely destroyed. We only share medical records with those that are involved in your care. You have the right to object to your medical records being shared.

You have the right to withdraw consent for record storage and to access your records at any time. You have the right to have any mistakes corrected and to complain to the Information Commissioners Office. To request access, please email info@gideaparkphysio.co.uk for assistance.
